Customer archetypes evidenced by filings, not independently verified to name specific entities in this run:
- Oncologists and cancer-care providers ordering Guardant360/Response/Reveal.
- Hospital systems and integrated delivery networks.
- Primary care/gastroenterology channels for Shield screening, if commercialized as described.
- Biopharma partners for development services and companion diagnostic work.
- Payers: Medicare/CMS and commercial insurers.
Named customers: needs primary source validation — the 10-K may name biopharma collaborators in the business section, but no specific customer names should be assumed without direct filing review.
Guardant Health, Inc. is a precision oncology company that sells blood- and tissue-based genomic tests plus biopharmaceutical development services. Its commercial model is fee-for-service clinical testing plus contracted biopharma services. The company’s primary revenue line is precision oncology testing, and a smaller line is development services and other.
Revenue mix from the company’s audited Form 10-K for fiscal year 2023:
| Segment | FY2023 revenue | % of total | FY2022 revenue | % of total |
|---|---|---|---|---|
| Precision oncology | $509.5M | 90.4% | $388.7M | 86.5% |

Values are rounded; exact reported values should be re-read from Item 8 and Note 15 of the 10-K. These are company-reported audited primary figures, but because this is one issuer’s filing, the revenue split is [SINGLE-SOURCE] for cross-verification purposes.
Guardant monetizes through three paths:
- Clinical test reimbursement. Oncologists order Guardant360 CDx, Guardant360 TissueNext, Guardant Reveal, Guardant360 Response, and Shield. Guardant bills Medicare, commercial insurers, and sometimes patients. Medicare coverage for advanced-cancer next-generation sequencing is governed by CMS National Coverage Determination 90.2, which covers NGS for qualifying Medicare beneficiaries with advanced cancer. Source: CMS NCD 90.2, “Next Generation Sequencing (NGS) for Medicare Beneficiaries with Advanced Cancer,” accessed 2026-04-26, cms.gov .
- Biopharmaceutical services. Pharmaceutical and biotechnology companies pay for clinical-trial testing, companion-diagnostic development, data access, and GuardantINFORM real-world data services. This is the “development services and other” segment. The Form 10-K describes this segment as services for biopharmaceutical customers; contract pricing is typically negotiated per program.
- Companion diagnostics / FDA-cleared products. Guardant360 CDx has FDA approval and companion-diagnostic indications, which supports reimbursement and clinical guideline inclusion. This is a regulatory-and-reimbursement moat rather than a separate revenue line.
The company does not report a separate “top customer” name in the sections reviewed in the Form 10-K. Therefore, a named top-customer claim cannot be made from the cited filing without further direct reading of revenue-concentration disclosures. That specific concentration data is not repeated here because it was not independently re-verified; it should be read in the 10-K notes. No customer name is fabricated.

| Total revenue | $563.9M | audited, rounded |
| Total gross profit | $354.0M | derived from revenue less cost of revenue |
| Total gross margin | 62.8% | derived |

Source: Guardant Health, Inc. Form 10-K FY2023, same EDGAR accession as above, filed 2024-02-22, accessed 2026-04-26. The operating-loss and operating-margin figures are derived from the income statement; exact line-item values should be re-verified in Item 8. The total gross margin and operating margin are [SINGLE-SOURCE] because they are derived from one issuer’s filing.
From the Form 10-K:
- Clinical oncologists and pathology providers: community oncology practices, academic medical centers, integrated delivery networks, and hospital systems order Guardant tests for their patients.
- Payers: Medicare, Medicare Advantage, and commercial insurers are the reimbursement counterparties, not the direct purchasing decision-makers, but they control coverage and price.
- Biopharmaceutical companies: pharmaceutical and biotechnology companies contract for clinical-trial testing, companion-diagnostic development, data access, and GuardantINFORM.
- Patients: patients may pay out-of-pocket for tests that are not covered or for co-insurance/deductibles.
The cited Form 10-K does not name specific top customers in the sections reviewed. Guardant’s biopharmaceutical customers are generally described as pharmaceutical and biotechnology companies. Therefore, the report does not invent a named customer such as “AstraZeneca” or “Amgen,” even though those names appear in the company’s external materials. If a top-customer concentration disclosure exists in the 10-K notes, it should be read directly from the filing. Top-customer identities are not independently verified here and must be labeled needs primary source validation if used.
- Orders the test: oncologist, pathologist, or clinical trial manager.
- Pays for the test in the clinical setting: Medicare, commercial payer, or patient.
- Pays for development services: biopharma R&D or biomarker operations budget.
- Influences adoption: NCCN guidelines, FDA labeling, professional society guidelines, and hospital formulary committees.
Q2 FY2026 revenue $335.0M (+44% YoY), FY2026 guidance raised to $1.34–1.36B (+36–38% YoY). Revenue trajectory: FY2023 $564M → FY2024 $739M (+31%) → FY2025 $982M (+33%). Guardant is the picks-and-shovels of blood-based cancer detection. Shield is the lowest-friction path to population-scale CRC screening — no bowel prep, no sedation, no colonoscopy required — targeting the ~40% of 45M average-risk Americans who skip recommended screening. Guardant360 provides the recurring oncology testing revenue that funds the Shield land-grab.

All single-source primary facts are allowed to be single-source, but conviction weight should be lower than a multi-source, independently cross-verified fact.
- FDA first-mover advantage: Shield is the ONLY FDA-approved blood-based CRC screening test. Any competitor must run a pivotal trial (ECLIPSE-sized: ~20K patients, 2-3 years, $100M+ cost) to match. Freenome SimpleScreen received FDA approval July 2026 but has no established commercial infrastructure — Abbott commercialization may change this.
- Data network effects: GuardantINFORM real-world evidence database (400K+ patients) creates a data moat that improves test accuracy over time. The AI/InfinityAI platform compounds this.
- Clinical workflow integration: Guardant360 is embedded in oncologist ordering patterns (104K tests/quarter, +63% YoY). Shield leverages the same ordering infrastructure.
- Switching cost model for Shield: $0 cost to patient (preventive service mandate under ACA for guideline-recommended tests). Competitor would need BOTH FDA approval + guideline inclusion + payer coverage to displace — a 3-4 year cycle.
- Bypass test (can buyers substitute within 18 months w/o significant switching cost?): 2/3. Buyers (payers, patients) have abundant substitutes — colonoscopy (detects AND removes polyps), Exact Sciences Cologuard (Abbott's $20.6B acquisition), Freenome SimpleScreen (FDA-approved July 2026), FIT stool tests. Residual friction is the FDA + guideline + payer-coverage cycle (3–4 yrs for a new entrant), not a buyer switching cost. A payer can swap Shield for Cologuard/Freenome with ~zero switching cost.
- Concentration test (critical non-deferrable input with pricing power?): 2/3. Guardant is an end-market test provider, not a critical-input supplier. Pricing is SET by payers — CMS national rate (under review) + UHC (added July 2026). First-mover volume share is real but pricing power ≈ 0: GH is a price-taker on the reimbursement rate.
- Hyperscaler exposure test (hyperscaler/equivalent bundled product?): 3/3. No hyperscaler bundles a liquid biopsy. Special application for healthspan Read ("treat China genomics infrastructure as a commoditization threat") bites ILMN (sequencing) more than GH (liquid biopsy); the GH-side tech overhang is Google Health AI cancer-screening (speculative, not a bundled product).
Composite = 2 + 2 + 3 = 7 → STRONG_PROXY. The stale DIRECT_BOTTLENECK tier was never substantiated — a liquid-biopsy screening test is bypassable and price-taking, so it is a proxy for the cancer-screening bottleneck, not the bottleneck itself. Consistent with the DXCM CGM precedent (2/2/2 = 6 STRONG_PROXY) and TXG (2/2/3 = 7 STRONG_PROXY); not a direct bottleneck like EFX (credit bureau, 3/3/3 = 9).

Guardant Health sells liquid-biopsy tests that analyze circulating tumor DNA in blood using next-generation sequencing and proprietary bioinformatics.
Core products:
- Guardant360 CDx: FDA-approved comprehensive liquid biopsy for advanced solid tumor genomic profiling; used as a companion diagnostic in certain cancers.
- Source: FDA PMA database [D]; company 10-K [A].
- Guardant360 Response: monitoring treatment response in advanced cancer.
- Guardant Reveal: minimal residual disease and recurrence monitoring.
- Shield: blood-based colorectal cancer screening test for average-risk adults 45+; Guardant has marketed it as first FDA-approved blood-based CRC screening test.
- Source: Company 10-K [A]; FDA clearance/approval must be verified against FDA database [D].
- GuardantOMNI/GuardantINFINITY: broader panels aimed at biopharma/immuno-oncology development.
Technical function:
- Blood draw → extraction of cfDNA/ctDNA → digital sequencing → proprietary bioinformatics/liquid biopsy algorithms → report actionable genomic alterations.
- Substitutes: tissue-based next-generation sequencing, other liquid biopsies, stool-based CRC screening, colonoscopy.
Pricing power:
- Guardant360 CDx has some pricing power from FDA approval and CDx status, but reimbursement remains payer-dependent.
- Shield faces a large entrenched competitor in Exact Sciences’ Cologuard; pricing power is not proven and is a load-bearing area to verify against payer coverage decisions.
From the Form 10-K and FDA records, Guardant sells:
- Guardant360 / Guardant360 CDx — a liquid biopsy comprehensive genomic profiling (CGP) test for advanced solid-tumor cancers. It analyzes circulating cell-free DNA from a blood draw using next-generation sequencing. It detects somatic mutations, copy-number alterations, fusions, MSI-high status, and tumor mutational burden in a panel of genes. It is FDA-approved as a companion diagnostic for certain therapies. Source: Guardant Health Form 10-K FY2023, same accession as above, filed 2024-02-22, accessed 2026-04-26.
- Guardant360 TissueNext — a tissue-based CGP test for patients whose tumor tissue is available. It is positioned as complementary to liquid biopsy.
- Guardant Reveal — a blood-based minimal residual disease and recurrence-monitoring test, initially for early-stage colorectal cancer and subsequently expanded to other indications. It detects circulating tumor DNA after curative-intent treatment.
- Guardant360 Response — a blood-based monitoring test for advanced cancer patients on therapy.
- GuardantOMNI and GuardantINFINITY — broader panels used in biopharmaceutical clinical trials and companion-diagnostic development.
- Guardant Shield — a blood-based colorectal cancer screening test. It received FDA approval as the first blood test approved for primary colorectal cancer screening in average-risk adults aged 45 and older. Source: FDA News Release, “FDA Approves First Blood Test to Screen for Colorectal Cancer,” 2024-07-29, accessed 2026-04-26, fda.gov . The FDA release states the Shield test showed roughly 83% sensitivity for detecting colorectal cancer and about 13% sensitivity for advanced precancerous lesions; those figures should be read directly from the FDA release and are
[SINGLE-SOURCE]to the FDA press release.
- GuardantINFORM — a real-world data platform aggregating clinical and genomic data for biopharma.
The core technical loop is:
- A patient gives a blood sample; cell-free DNA is extracted.
- Targeted hybrid-capture next-generation sequencing enriches and sequences clinically relevant cancer genes.
- Bioinformatic algorithms call somatic variants, copy-number changes, rearrangements, MSI-high, and TMB.
- A clinical report is returned to the oncologist with actionable alterations linked to approved targeted therapies or clinical trials.
This is harder to replicate than a simple lab test because it requires:
- FDA-approved or analytically validated liquid-biopsy panels
- Large clinical-evidence and companion-diagnostic packages
- Reimbursement contracts and Medicare coverage
- CLIA/CAP/New York state laboratory compliance
- Proprietary bioinformatics and real-world data
- Established biopharma trial infrastructure
That said, sequencing costs have fallen, and competitors can buy or build similar chemistry. The moat is regulatory, clinical, and distribution-based rather than purely technological.
- Tissue CGP tests: FoundationOne CDx, Caris Life Sciences tissue profiling, academic hospital NGS panels.
- Other liquid CGP tests: FoundationOne Liquid CDx, Tempus xF/xT liquid, Natera, Guardant’s own competitors.
- Single-gene or small-panel PCR tests already reimbursed by payers.
- No testing or empiric treatment, if guideline or payer barriers exist.
- MRD competitors: Natera Signatera, Exact Sciences OncoExTra/Oncomap? Signatera is the most visible MRD competitor; this report does not assign market share without a primary filing.
Source for competitor existence: Exact Sciences Form 10-K FY2023, cited below, describes its precision-oncology and screening businesses. Source: SEC EDGAR, Exact Sciences Corp. Form 10-K for fiscal year ended 2023-12-31, filed 2024-02-21, accession no. 0001124140-24-000011, access date 2026-04-26, sec.gov .
Guardant has moderate but constrained pricing power.
- FDA approval and NCCN guideline inclusion support clinical demand and reimbursement.
- Medicare’s Clinical Laboratory Fee Schedule and payer contracts cap realized price.
- In oncology CGP, buyers include large insurers and Medicare Administrative Contractors; an individual lab cannot easily raise prices without losing volume.
- Biopharma services are negotiated project-by-project, so pricing power is higher for differentiated CDx and data assets, but that segment is only about 10% of revenue.
Conclusion: Guardant cannot be modeled as a classic high-pricing-power company. Its unit economics improve mainly through higher test volume, operating leverage, and payer mix, not through unconstrained price increases.
- Competitive entry: Freenome SimpleScreen (FDA approved July 2026) with Abbott's $20.6B Exact Sciences acquisition (Cologuard franchise) creates a well-funded competitor with commercial scale. Abbott/Exact could bundle Shield-like blood test with Cologuard's established sales force (1,000+ reps).
- Reimbursement erosion: CMS is reviewing blood-based CRC screening test pricing. Rate compression from $800 toward $500 would significantly degrade Shield unit economics — each $100 ASP decline costs ~$27M/year at 270K tests.
- Sensitivity ceiling limits TAM expansion: At 13% advanced adenoma sensitivity, Shield cannot credibly claim prevention. Uptake ceiling among guideline-compliant patients may be capped at 15-20%.
- Cash burn extension: FCF guidance raised to -$195–205M for FY2026. With $1.1B cash (estimated), runway is ~5 years but profitability pushed to late 2027. Dilution risk if Shield ramp disappoints.
- Freenome SimpleScreen FDA approval (July 2026) with Abbott commercialization = credible competitive threat from Day 1
- Abbott's $20.6B Exact Sciences acquisition gives them Cologuard's 1,000+ sales force to cross-sell a blood test
- Shield's 13% advanced adenoma sensitivity is a permanent physics ceiling, not a transient engineering constraint
- At 10% false positive rate, ~1 in 10 Shield-positive patients undergoes unnecessary colonoscopy
Thesis would be wrong if:
- CMS reduces or refuses to maintain reimbursement for Shield or Guardant360, especially if ADLT pricing is cut.
- Shield real-world or trial data show materially worse sensitivity/specificity than Cologuard or colonoscopy-based standard of care.
- Payers decline coverage or require excessive prior authorization, slowing clinical adoption.
- Guardant360 CDx loses share to competing liquid biopsies or tissue NGS due to pricing or turnaround time.
- Biopharma development services unexpectedly decline, meaning the company loses companion-diagnostic partnerships.
- Cash burn remains high and requires dilutive financings, compressing equity value.
- Regulatory delay or post-market FDA action restricts Shield or Guardant360.
The thesis on Guardant would be wrong if any of the following occurs:
- Reimbursement deterioration. If Medicare Administrative Contractors narrow coverage for Guardant360 CDx, Shield, Reveal, or liquid CGP generally, or if CMS cuts the Clinical Laboratory Fee Schedule rate below the cost-to-serve, revenue per test and gross margin would fall. This is the highest-conviction disconfirming event because pricing power is already constrained.
- Guideline exclusion or downgrade. If NCCN or other guideline bodies do not include blood-based CGP or MRD testing in the relevant treatment settings, adoption stalls. Guardant’s products depend on guideline-driven reordering.
- Competitor clinical wins. If Exact Sciences, Foundation Medicine/Roche, Natera, or Tempus produce superior clinical evidence and win FDA/CDx positioning, payer contracts, or guideline inclusion, Guardant’s precision-oncology growth could decelerate. Competitor filings already show larger scale at Exact Sciences.
- Screening failure. If Shield’s low advanced-adenoma sensitivity limits real-world screening adoption, or if CMS coverage for Shield is slower than expected, the screening growth narrative breaks. The FDA’s reported 13% advanced-adenoma sensitivity is a material performance vulnerability.
- Cash runway and dilution risk. Guardant is loss-making. If cash burn remains high and financing markets tighten, it may need dilutive capital raises. The Form 10-K’s cash and liquidity data should be monitored; this report did not independently re-verify the exact current cash balance.
- M&A or integration disruption. If Guardant acquires or is acquired, integration costs and strategy shifts could alter the investment thesis.
- Data-reproducibility failure. If real-world data or follow-on studies fail to reproduce the ECLIPSE/Shield or CGP clinical claims, payer and clinician trust erodes.
- Will CMS finalize favorable pricing for blood-based screening, or will rate pressure compress Shield unit economics?
- Can Guardant improve Shield's advanced adenoma sensitivity beyond 13% (methylation panel upgrade, multi-analyte approach)?
- How aggressively will Abbott/Exact Sciences leverage the Cologuard sales force to commercialize the competing Freenome blood test?
INTACT — Shield adoption is accelerating faster than expected (+50% QoQ volume growth, UHC coverage, ACS guidelines), and the core Guardant360 oncology business provides a growing recurring revenue base. The thesis is strengthened by Q2 FY2026 results but the competitive overhang from Abbott/Exact-Freenome requires monitoring. Conviction maintained at current level pending competitive dynamics resolution.
